Join our Team! We have an exciting new opportunity for a Housing Case Manager in our Supported Housing Program in Tarrytown, NY. Base Annual Salary Range: $44,000 to $46,000.

SUMMARY of DUTIES:
Our Supported Housing units offer the opportunity for clients to work with a housing specialist to develop skills such as managing personal finances, negotiating with their landlord, and accessing community resources. The Housing Case Manager is responsible for providing housing-related case management services to residents of the agency’s housing units. The Case Manager will support recovery by providing services that reflect our deeply held values, including the recognition of individual rights of self-determination, choice, shared decision-making, and collaboration. When possible, he/she will utilize evidence-based practices, always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES:
1. Provide a housing-related psychiatric rehabilitation assessment for each prospective tenant.
2. Assist the Program Director in tenant selection.
3. Develop a housing-related rehabilitative support plan for each tenant.
4. Visit tenants both on a regularly scheduled basis and as needed.
5. Work with tenants individually and in groups to help them live together.
6. Supervision, teaching through demonstration, and training in public transportation for skill acquisition.
7. Follow up with tenants regarding their treatment plans (including medication), rehabilitation programs, work, or education.
8. Develop organizational ties to resources that the tenants, including mental health services and general resources, may need.
9. Serve as liaison between the supported housing program and other service providers.
10. Provide crisis intervention in psychiatric and situational crises requiring professional intervention.
11. Document client progress to maintain a permanent record of client activity according to established methods and procedures.
12. Provide treatment contacts in the community.
13. Other related duties as assigned.

REQUIRED KNOWLEDGE and SKILLS:
1. Demonstrates the ability to act responsibly in stressful situations to undertake emotionally difficult aspects of working with adult populations.
2. Written and verbal communication skills sufficient to effectively provide direction and counseling to clients and maintain required documentation.
3. Must be capable of independently and professionally assessing the physical and emotional well-being of the client and ascertaining the adequacy of the physical environment and treatment/educational/vocational programs.
4. Knowledge of the common challenges associated with mental health conditions.
5. Ability to independently organize time and prioritize responsibilities to plan and carry out a demanding schedule.
6. Understanding the impact of culture on mental health practices.
7. Familiarity with and ability to use an Electronic Health Record system.

QUALIFICATIONS:
1. The Housing Case Manager must have at least 1-year experience, be knowledgeable in the planning and provision of social support, and be able to provide crisis intervention.
2. A Bachelor’s Degree in a Human Services field is preferred.
3. Basic Computer knowledge is required.
4. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
5. Confidentiality must be maintained on tenant information.
6. The Housing Case Manager must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
7. The Housing Case Manager must maintain current New York State Defensive Driving certification.
8. The Housing Case Manager must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
9. The Justice Center must clear the Housing Case Manager for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
10. The Housing Case Manager must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
11. The Housing Case Manager must be legally eligible to work in the United States.
