Licensed Practical Nurse
Westfield Family Physicians, PC is a primary care group serving a patient population of about 12,000 patients in two locations: Westfield, NY and Sherman, NY. Read more about our practice at http://westfieldfamilyphysicians.com/
We are seeking 2 full-time Licensed Practical Nurses to join our team. A successful candidate must be eager to become an active part of the care team. The nurses at Westfield Family Physicians work closely with the healthcare providers to provide care across the lifespan. Our goal is to help our patients live healthy lives.
Nursing staff members are responsible for performing such duties as taking vital signs, giving immunizations, reconciling medications, and educating patients. They also interact with patients and insurance companies over the phone. They are responsible for assisting the providers in keeping patients up to date with their preventive care and chronic care management. They play a key role in helping patients navigate the health care system.
Skills and requirements:
- Current NYS Nursing license
- Experience preferred
- CPR certification
- Comfortable using computerized systems / electronic health record (EHR)
- Good communication skills
Duties and Responsibilities:
- Check patients into the exam room, obtaining pertinent vital signs
- Update the patient medical, family and social history
- Reconcile medications as needed
- Perform any indicated tests, such as EKGs, spirometry, diabetic foot exams, etc.
- Perform simple lab tests such as pregnancy tests, rapid strep tests, protime tests, etc.
- Administer all vaccines as recommended by the ACIP
- Accurately document all information in Medent EHR
- Maintain strict confidentiality of all patient information
- Reach out to patients for needed services and follow up
- Work closely with healthcare providers to help the patient manage their particular condition
- Participate in morning huddles and communicate relevant information during morning huddles
- Communicate effectively with patients and staff
- Conduct oneself in polite and courteous manner to co-workers and patients
- Attend scheduled meetings and trainings
- Various other tasks as assigned by team leader and/or other management staff
Physical Requirements:
- Standing, walking for a 10 hour day
- Ability to lift 5 lbs.

- Hourly wage rate negotiable and dependent on work experience
