Job Description
Seeking an experienced Health Home Care Management (CMA) program coordinator to direct our growing Care Management team and implement & achieve the overall strategic goals of the organization.
Responsibilities include:
·Direct and coordinate the Health Home Care Management program
·Implement program policies and procedures, ensuring compliance
·Support the program's successful development and growth
·Oversee the use of technology and software programs for the organizations operations
·Supervise a unit of Care Managers with a mixed acuity caseload ranging from 20-40 clients each, providing support and direction
· Participate in the recruitment of new staff by interviewing applicants and providing input on hiring
· Orient and conduct regular training for CMs on casework methods and techniques to ensure each possesses the required competencies to successfully perform their job
· Conduct meetings with CMs on an individual basis to discuss specific case problems and work performance issues; provides written and verbal evaluations on an ongoing basis; counsel and if needed, discipline employees on work performance issues as necessary; seek advice from the Director of Client Services and/or Human Resources
· Conduct weekly/biweekly supervision with each Care Manager to review case details as it relates to case needs, overall well-being, and progress of each family
· Review timeliness, completeness, quality, and appropriateness; approve and monitor service plans with Quality Assurance and Utilization
· Ensure and track that staff is attending all training as required and remain updated with any needed certifications (i.e.; CANS-NY)
· Review all documentation to ensure service provisions are in compliance with DOH and the designated Health Home regulations
Minimum Requirements:
· Master’s Degree in Human Services, Developmental Services, Social Work, Nursing, Public Health, or related fields
· 3-5 years of administrative or clinical supervisory/managerial experience
·6+ years of Health Home Care Management experience, preferably children's
· Knowledge of best practices working with children and families in health, and preventive wellness care
Job Type: Full-time
Pay: $85,000.00 - $100,000.00 per year
